Heather Rutz, RN, CCM, CDP
Care Coordinator
Dementia Support Program
I am a registered nurse with over 18 years of experience providing case management services to a diverse population of clients. I have effectively coordinated services for adult and elder patients having a broad spectrum of injuries and illnesses to help them achieve the highest quality outcomes during their inpatient physical rehabilitation hospitalizations. I have provided comprehensive case management services for injured workers as well as adults and children with mental health diagnoses. Advocacy and support for individuals of all ages with intellectual disabilities has always been a passion of mine, both personally and professionally, and I have been lucky to do so in a variety of settings over the years including, but not limited to, protective, recreational, residential, educational and vocational services. I earned my bachelors degree in rehabilitation services from Wright State University and my nursing degree from Roanoke Chowan Community College. I am a nationally Certified Case Manager (CCM) and a Certified Dementia Practitioner (CDP).
