Important Information About Your First Visit

1. Open the PDF file with Adobe Acrobat Reader DC
2. Select Sign -> Fill & Sign.

3. If asked “Who needs to fill and sign?”, answer “You” and select the Fill and sign button.

4. With the Add text tool selected, scroll through the document and text boxes will appear in the blanks on the form.

5. Type the information into the various fields and add checkmarks to appropriate boxes.

6. Once the form is complete, save the file to your computer.
7. Print or send the file as an email attachment to NeuroPsych Center of Greater Cincinnati for processing.
